The bloated face of actor Jared Leto in his portrayal of Mark David Chapman is like the opposite of the poster for The 40-Year-Old-Virgin, and quite disturbing.

Nobody likes getting something in their eye. Having an entire hand in there is especially unsettling.
One Missed Call

More disturbing eyes. Or, more precisely, the lack of eyes in this case. The grin makes the design even creepier.
The Ruins
There are a number of disturbing posters for this film featuring killer plant life. This one, featuring the girl screaming, is our pick for the creepiest of the series.
Saw V
What do you do when the main killer of a horror film franchise is dead? Feature someone else wearing his face, of course! Nasty indeed.
